The physicians and staff at the Cedars-Sinai Kerlan-Jobe Institute are ready and practiced in guiding your referred workers’ compensation cases by providing quality and responsive customer service to our referral partners. Our workers’ comp staff cater specifically to nurses, adjusters, employers and occupational medicine physicians. Our department focuses on getting your injured workers scheduled promptly while at the same time keeping the employer’s and insurance carrier apprised of the patient’s treatment plan, progress, and status.

To request work status, next appointment status, copy of the last chart note and/or RU-90, call our direct status line at 310-665-7200. Ext 6600.

To request a worker’s comp appointment, please submit the following information:

  • Workers’ Compensation Insurance Authorization Letter addressed to physician (employer’s information, workers’ compensation insurance carrier information, claim number, date of injury, Adjuster information)
  • Patient demographic sheet (patient information)
  • Current Medical Records, including diagnostics (related to injury), and Operative reports, if available.

Case Appointment Scheduling

If you are inquiring about your initial appointment for a work-related injury, your Case information must FIRST be approved and registered in our system, submitted by your employer or your employer’s designated representatives (or you) and must include case information documents listed above.

Processing of your case may take up to 6 weeks from receipt of Case information including both physician authorizations along with your medical records prior to contacting you to schedule your appointment. Once your case is accepted and registered in our system, we will contact you directly for your appointment.  For (acute) serious injuries, please call 310-665-7280 to discuss escalating our process.

If you already have records and/or prior authorization, you can help expedite the process by faxing case information documents to number 310-216-9389.  Include cover letter when faxing with contact information and case number.
